For CHMM Program Applicants:
All applications must be accompanied by the non-refundable $100 application fee, a signed Application Consent Statement, and a detailed work history or resume. An official, original college transcript and two reference evaluations are also required prior to final approval, but they may arrive separately from the application.
For Those Who Have Been Notified that They Passed the CHMM Examination:
For CHMP Program Applicants:
