CHMM Application for Recertification RECERTIFICATION DUE IN 2010 for the 5-Year Period of January 1, 2005 through December 31, 2009.
CHMM Applications. Apply online here (If you've never applied before, please select the link for "First Time Users" to set up your account.) All applications must be accompanied by the non-refundable $125 application fee, which may be paid online using a credit card, or by mailing in a check or money order. Once you have submitted your application online, use the links below to access the forms required for supporting documentation. You must also submit a detailed work history or résumé, and an official, original college transcript. Supporting documentation may arrive prior to or after submitting an application, but ALL of these items are required before IHMM will issue a final approval to sit for the CHMM examination.

All applications must be accompanied by the non-refundable $50 application fee, which may be paid online using a credit card, or by mailing in a check or money order. Once you have submitted your application online, use the link below to access the Consent Statement form, which is required before your application will be reviewed. You must also submit an official, original college transcript, evidencing your degree earned (unless you are still in your final semester). Both of these items are required before IHMM will issue a final approval to sit for the HMMT examination.
